These prices apply to German and international patients alike. Lipedema (lipoedema, German: Lipödem) is a chronic disease and the surgery a medical procedure, but Germany has no fixed price for private treatment; every practice sets its own. What the session price includes, and what it does not

It excludes services provided by third parties such as the anaesthetist, the compression supplier, the lymphatic therapist and the pharmacy.

The cost of a complete lipedema treatment
A complete lipedema treatment realistically costs 9,000 to 26,000 EUR with Dr. Fetai, because the German S2k guideline foresees 1 to 4 sessions for the legs and 1 to 2 for the arms, and each session costs 4,500 to 6,500 EUR.

German insurance context 2025: what applies, and what does not apply to you
Since 9 October 2025, liposuction for lipedema has been a regular benefit of German statutory health insurance in all stages (G-BA decision of 17 July 2025); since 1 July 2026 statutory physicians bill EBM code 31095 (605.17 EUR: lower leg, upper arm, elbow, forearm) or 31096 (769.14 EUR: thigh, knee) per area. Dr. Fetai runs a private practice without statutory billing, so every patient pays privately.

Why does lipedema surgery cost what it costs?
What is behind the price of a session
The price of a lipedema liposuction session is made up of the time of the operating physician and the team, the outpatient operating facility, the WAL equipment and consumables, the tumescent solution and medication used during the procedure, and the follow-up appointments over three months. In a private practice these costs are calculated per session and set out in a written cost plan before you book, so that nothing is added afterwards.
The largest single item is time. A WAL session for lipedema is not a quick procedure: the tumescent solution alone needs 60 to 120 minutes to take effect, the aspiration of a large region takes one to two hours, and preparation, resting and discharge add to it. Add the consultation and examination beforehand and four follow-up appointments afterwards, and one session represents a full working day of physician time spread over three months. Practices that quote a much lower price usually either include fewer of these elements or treat a smaller area.
The second item is equipment and consumables. The Body-Jet system uses single use cannula sets and sterile tumescent solution in litre volumes. Sterile drapes, dressings, the first compression garment and the medication given during the procedure are consumed per session. The third item is the facility: an outpatient operating room in Germany must meet hygiene and safety standards that are inspected, with a recovery area and emergency equipment.
What the price does not contain is a margin for advertising promises. German law on health advertising (Heilmittelwerbegesetz) forbids promises of success and superlatives, and the practice follows this in every language. No result is guaranteed, and lipedema, as the German guideline states, cannot be cured by liposuction.
How does the written cost plan work, and what if the plan changes?
The cost plan: fixed price before booking
After the consultation you receive a written cost plan in English that names the areas of the session, the price, what is included, the optional general anaesthesia and the excluded items. The price in the cost plan is the price you pay; it changes only if you and Dr. Fetai agree to change the extent of the session before the surgery date, never afterwards.
The cost plan is based on the examination. For international patients the first assessment is made in a video consultation, and the cost plan sent after it is marked as preliminary; it is confirmed at the in person examination in Düsseldorf before surgery. In the experience of the practice the preliminary plan is rarely changed, because the video consultation with photographs and measurements gives a good picture of the extent of the lipedema. Where a change is necessary, for example because a region turns out to be larger or smaller than expected, this is discussed and agreed with you before anything is done.
Payment terms are stated in the cost plan and agreed before the surgery date; international patients usually pay by bank transfer in euros. A treatment contract and the written information about risks are signed at the in person appointment with sufficient time to consider them, not on the morning of surgery. Every session is invoiced separately, so you decide session by session whether and when to continue, and there is no obligation to book the full treatment at once.
If Dr. Fetai concludes at the in person examination that surgery is not advisable in your case, you pay for the consultation only, and he will say so plainly.
Can I get reimbursement from my insurance at home?
Reimbursement from foreign insurers and health services
Whether an insurer, a private policy or a national health service outside Germany reimburses part of a lipedema liposuction in Germany depends entirely on the rules of that insurer, and only they can answer it. The practice supports a request with a detailed invoice, a medical report, the findings of the examination and, on request, an English summary; it does not promise or estimate any reimbursement.
Patients from EU countries sometimes ask about planned treatment abroad under European rules; these usually require prior authorisation from the home insurer and a treatment that would be covered at home, which is a question for that insurer, not for the German practice. Patients with private health insurance sometimes have policies that cover medically indicated treatment abroad up to a limit; the policy wording decides. Patients from the United Kingdom, the United States, the Gulf region or the Balkans most often pay privately without reimbursement and plan the cost accordingly.
What helps in every case is documentation. A confirmed diagnosis of lipedema from a physician at home, a record of conservative therapy and its result, and a clear medical indication in the report from Düsseldorf make a request stronger. The German statutory framework can be quoted as context: the G-BA decision of 2025, the LiPLEG trial with its twelve month pain data and the S2k guideline show that liposuction for lipedema is an established, medically indicated treatment in Germany, not a cosmetic procedure.
For German taxpayers, costs of a medically indicated treatment can be claimed under section 33 of the Income Tax Act (EStG); whether comparable rules exist in your country is a question for a tax advisor at home. The invoices state the medical indication.
How should I budget for the whole treatment, including travel?
A realistic budget for an international patient
A realistic budget for an international patient consists of the session prices (4,500 to 6,500 EUR each), general anaesthesia if chosen (500 to 1,100 EUR per session), compression garments, external lymphatic drainage and medication, plus travel and accommodation for each stay of about 7 to 10 days per session. Only the session prices and the anaesthesia range are fixed figures from the practice; the rest depends on your choices and your country.
Start with the number of sessions from the consultation, for example two for the legs and one for the arms, and multiply by the price in the cost plan. Add general anaesthesia only if you choose it; most patients of Dr. Fetai are treated under tumescent local anaesthesia, which is included. Add compression garments: ready made garments for the first weeks and, if prescribed, custom made flat knit garments, priced by the supplier. Manual lymphatic drainage is useful after surgery but optional; therapists in Düsseldorf and at home charge their own rates.
Travel and accommodation vary widely. Düsseldorf has accommodation in every price category within walking distance of the practice near the main station. A companion is strongly recommended for the day of surgery and the day after, so budget for two people for at least the first few days of each stay. Return flights should be booked with flexibility, because the decision about fitness to fly is made individually by Dr. Fetai and may lead to a later departure than planned.
Finally, keep a reserve: a longer stay, additional garments or medication should not create financial stress. Planning one session per trip over one to two years spreads the cost and gives each area time to heal.
Why are some quotes for lipedema surgery so much cheaper, and what should I ask?
Cheap quotes: what to look for before you decide
A much cheaper quote for lipedema liposuction usually differs in one of five respects: technique, surgeon, anaesthesia, follow-up or area. Before you decide, ask which technique is used and whether it is wet and tissue sparing, who exactly operates and how many lipedema cases this person has documented, whether the anaesthesia is included, how many follow-up appointments are included and who is responsible if a complication occurs after you have gone home.
Technique first. Dry liposuction without prior tumescent infiltration is prohibited under the German G-BA directive because of the higher risk to lymph vessels; a quote that does not name the technique deserves a question. Surgeon second. In Germany, the directive requires documented experience of at least 50 independent lipedema liposuctions for statutory treatment; this is a sensible benchmark in any country. A clinic that cannot tell you who will operate on you, or where the person who consults is not the person who operates, is organised differently from the practice of Dr. Fetai, where one physician does both.
Anaesthesia and follow-up are where hidden costs most often appear: some quotes list the surgeon's fee only and add anaesthesia, facility fee, garments and each follow-up separately. Ask for a written list of what is included and compare it with the checklist on this page.
None of this means that Germany is the only place for lipedema surgery; good surgeons exist in many countries.
